This questionnaire is no longer active. It is an example of a survey done in Spring 2002. Your responses will not be recorded. If you have questions, please refer to the note that accompanied the questionnaire.

Your candid responses to the following questions are important as we design the next phase of the Borrow Direct service.

In this section, you are asked first to think about the Ideal Service through which you could borrow materials from the
collections of other research libraries, and indicate the extent to which you agree or disagree that the ideal service should
possess the feature. If you feel a feature is “of no importance” please mark the number “1” for “strongly disagree.”If you
feel a feature is “of highest importance” for the ideal service, mark “7” for “strongly agree.”

Then, from your experience, please indicate the extent to which you believe the Borrow Direct service has the feature
described, by marking a number on the scale provided.Marking “1” means you “strongly disagree” that the Borrow Direct
has the feature and marking “7” means you “strongly agree” that it does have the feature. From either perspective, you may
mark any of the numbers in the middle of the scale to reflect your opinions. If you have “no opinion,” please skip the statement.

There are no right or wrong answers. We are interested in a number that conveys your feelings regarding excellent service quality
for interlibrary borrowing.Your responses will be strictlyanonymous.
