ORBIS Heading Change Request Form

Library staff may use this form to report problems associated with name, subject, series, and uniform title headings in ORBIS, including:

• split files in heading index displays
• headings that have been assigned incorrectly
• headings that need additional cross references
• duplicate or conflicting authority records
• report problems with linked fields in records that contain nonroman script data (linked fields begin with $6)

Staff may also use this form to request the addition of a death date to an "open-ended" heading if:

• they have received a patron complaint
• the person represented by the heading has strong associations with Yale or New Haven
• the person represented by the heading will be the subject of a local event (conference, lecture, etc.)
