Notes:
Forms created to use at the local level for program management purposes. Promotes consistency across program since numerous people are involved.
Assessment: utilized survey tools for library faculty, students, and HSC faculty and staff
Evaluation: most overlooked aspect in education services but one of the most important. Must be emphasized and included as standard procedure from the outset. We distribute an evaluation form to each attendee for every class.
Program - overall picture; includes anecdotal evidence, feedback, written evaluations, etc.
Content - done each class
Instructor - done each class, also with peer feedback. Formative in nature; i.e. non-judgemental, focus on changeable behavior, and supportive
