SCOPA Mentoring Program Survey

The SCOPA mentoring program has been in effect since 1999. As someone who has participated in the program, you can help us determine how effective the program has been and whether there are changes we need to make to improve upon the program. The survey should only take a few minutes of your time. Your answers will not be shared with anyone outside of SCOPA. Also, we realize you might prefer to remain anonymous; if so, please simply omit your name and any other identifying information (email, department, phone) from the survey.

The survey is divided into 3 sections. Once you have indicated (below) how you participated in the program, you should automatically be taken to the appropriate starting point for the survey.

How did you participate in the mentoring program?
