Breach cure procedures

Dear Liblicense,

We have had two reports from publishers in the last 2 months 
about unusual activity or an open proxy.  We cured both, but it 
is clear we need better written procedures.  Does any one have 
procedures that they would be willing to share or can you direct 
me to a website with suggestions?
